How to report incorrect or missing information in your record
If you find missing or incorrect information in your record, for example, a document in someone else’s name, please contact us as soon as possible using the Help line on 1800 723 471 (option 1).
You can also remove the incorrect information from your record.
Inform your doctor or healthcare professional
Healthcare professionals must take reasonable steps to make sure your health information is accurate, complete, up-to-date, relevant and not misleading. You can ask your doctor to correct health information in your record.
If your doctor or healthcare professional corrects your information, you can also ask that they notify any other people affected.
